What is my responsibility for chiropractic care?

We're dedicated to keeping your spine healthy and active. We believe in transparent insurance billing and want you to understand our process.

Your first visit includes a comprehensive exam and your initial treatment. This treatment typically involves a chiropractic adjustment for your spine and extremities, along with manual therapy, therapeutic modalities, and/or exercises. Based on the exam, we'll create a personalized treatment plan just for you. Our goal is to realign your spine, rebalance your posture, and address muscle spasms and tension.

We'll collect your portion of the cost at the time of your visit. If you haven't yet met your deductible, this will include that amount. We're in-network with most insurance plans and will collect the contracted rate for the services you receive.

Before your exam and treatment, Dr. Cheng will explain each procedure and ask for your consent. Please don't hesitate to ask any questions or express any concerns you may have beforehand. We're committed to making your experience as stress-free as possible.

DynaRom MyoVision at Active Spines Chiropractic

As an attorney, what do you do when you have a client that you know has lasting damage and pain, but has received a negative IME or their pain doesn’t show even with advanced imaging?
Normal range of motion assessments only show how far the muscles are able to extend and often appear normal, even though a patient is still experiencing pain. When you are involved in a car accident, it is very important that you follow up with your chiropractor or medical doctor for a detail check up at a timely manner. You might feel neck pain, back pain, headache, body soreness, tightness, fatigue, sleeplessness, dizziness, difficulty concentrating, changes in your daily routine, or more. [5] Detail history taking regarding the speed of impact, types of car involved, direction of impact, driver's hand, arm, leg, body and head position, aware or unaware of impact, seat belt fastened or not, road condition, weather and more are needed to determine the mechanism of injury due to motor vehicle accident. 

Motor vehicle accidents (MVA) may result in intractable disability. Post traumatic stress disorder (PTSD) can be as a potential cause. [2] Some patients experience afraid of driving on the road and gets nervous pretty easily. It is important to follow up with a medical doctor or a psychotherapist to evaluate and treat this condition. Cognitive-behavioural intervention and antidepressants may improve coping, ease fear, and reduce the impact of pain. [2] Patient should keep a detail record of their daily activities after the accident such as their sleep pattern, work, hobby, taking care of their child, and driving [3], [4]. Keeping good records of your improvement in daily activity level is a good way to monitor your progress.

Chiropractor Documentation of Care in Washington State

Below is the updated Washington State law regarding Documentation of Care for Doctor of Chiropractic:

Documentation of care.

A doctor of chiropractic must keep complete and accurate documentation on all patients and patient encounters. This documentation is necessary to protect the health, well-being and safety of the patient.
(1) The patient record must detail the patient's clinical history, the rationale for the examination, diagnostic or analytical procedures, and treatment services provided. The diagnosis or clinical impression must be contained in the patient record, not merely recorded on billing forms or statements. Subjective health status updates, whether or not symptoms are present, must be documented for every patient encounter.
(2) Documentation for the initial record must include at a minimum:
(a) The patient's history;
(b) Subjective presentation;
(c) Examination findings or objective findings relating to the patient's presenting condition;
(d) Any diagnostic testing performed;
(e) A diagnosis or impression;
(f) Any treatment or care provided; and
(g) Plan of care.
(3) Reexaminations, being necessary to monitor the progress or update the current status of a patient, must be documented at reasonable intervals sufficient to reflect the effectiveness of the treatment. Reexaminations must also be documented whenever there is an unexpected change in the subjective or objective status of the patient. Reexamination documentation must include the subjective presentation and objective findings. This documentation shall also reflect changes in the patient's care and progress and in the treatment plan.
(4) Documentation between examinations must be recorded for every patient encounter. Documentation must sufficiently record all the services provided, as well as any changes in the patient's presentation or condition. The region(s) of all treatment and, if applicable, the specific level(s) of chiropractic adjustments must be recorded in the patient encounter documentation.
(5) Patient records must be legible, permanent, and recorded in a timely manner. Documentation that is not recorded on the date of service must designate both the date of service and the date of the chart note entry. Corrections or additions to the patient's records must be corrected by a single line drawn through the text and initialed so the original entry remains legible. In the case of computer-organized documentation, unintended entries may be identified and corrected, but must not be deleted from the record. Errors in spelling and grammar may be corrected and deleted.
(6) Correspondence relating to any referrals concerning the diagnosis or treatment of the patient must be retained in the patient record.
(7) Patient records should clearly identify the provider of services by name, initials, or signature. If the chiropractor uses a code in the documentation, a code legend must be made available upon request.

I went to a mini seminar provided by Graham Lundberg & Peschel law firm yesterday. Here I would like to share how to file your personal injury protection coverage application properly.

Firstly, you should report the accident to your insurance company or the driver's insurance company. You need to report the basics of the incident such as how the accident happened, and where you were injured such as neck pain or back pain. If there are questions you are not sure or do not know, just say "you do not know".

Secondly, you will receive a PIP application in the mail from the car insurance company within 10 business days. This package contains important information. Please do not throw them away as if they are junk mail. You should read the PIP application coverage thoroughly, fill out the forms and mail them back to the car insurance company. If you have questions or concerns about how to fill out the application you should seek legal advice from an attorney.

Lastly you should seek medical or chiropractic care at a timely manner to ensure proper recovery from your injuries. Why Am I Here? What Make You Decide to Be a Chiropractor?

      Since I grew up in Taiwan, Chiropractic is not popular back home. I did not decide to be a chiropractor when I was young or I had seen a chiropractor prior due to sports injuries, car accident injuries, neck pain or back pain. As a matter of fact I have never heard of the word "chiropractic" until when I was a junior or senior in college through my dad. My dad goes to Japan from time to time for work or visiting friends. One time he was seen by a chiropractor there due to his chronic low back pain. Later he told me about his experience and wanted me to find out more about it.

      I had no clue what chiropractic is and was very skeptical about how it can help back pain by adjusting the spine. At that time I was studying Athletic Training and helping athletes with various sports injuries through rehabilitation exercises and therapeutic modalities. With all the classes I had taken, none of them mentioned chiropractic care as a treatment option. But I decided to find out more by contacting schools in the USA and visited local chiropractors in Taiwan. I received  admission and curriculum information from chiropractic schools. After reviewing all the information that really got me excited because the classes looked quite challenging and interesting comparing to going to graduate school doing research. I had been studying for GRE in Taiwan, took a couple tests and had good scores for admission to graduate school at that time. But I prefer hands on, clinical approach, and more patient interaction. After long discussions with my parents and teachers I decided to go to chiropractic school at National University of Health Sciences in Lombard IL, instead of graduate school in sports medicine and the rest is history!

At the end of May, 2012, I finally finished the 4 module seminar presented by Dr. Dan Dock. Here is the certificate of completion from CDI. It looks pretty good, isn't it. I have been taking Dr. Dan Dock's seminar since 2007. Overall it has been a great experience. The seminar keeps me up to date with studies, researches and different treatments in auto accident injury care.

Your first visit in general:

A detailed history of your car accident bodily injuries will be taken. We have to ensure the mechanism of the injuries correlate your clinical presentation and findings. Most likely diagnostic imaging such as x-ray for your neck and back will be done in the office. Mild gentle care will also be provided on the first visit such as ice therapy and gentle chiropractic care to relief pain. Seeking medical care is also recommended for pain control. Usually your medical doctor will prescribe pain killer, anti-inflammatory and/or muscle relaxant.

Yard work, Back pain! What' the solution? | Bellevue Chiropractor

Low back pain after yard work. Is there help for that?


I was doing yard work for 4 hours yesterday since the sun was out. I shoveled 2 yards of mulch. That was way more than I expected. And my back was not too happy this morning. What should I do?

Besides paying attention to my posture while pushing wheel barrel, shoveling dirt, bending and lifting, I also took several mini breaks in between. I stretched out my back, legs and thighs. I drank extra water. I also paid extra attention to my posture when sitting and driving. I put a pillow on my low back while sitting down and did not sit on a soft chair. I sat all the way back and rest my back again the backseat while driving last night. All these were intended to maintain the low back lordosis, curvature. With bending and twisting my back while shoveling, I need to restore the natural curvature in my low back with self care methods mentioned above.

After doing all those stretching exercise and maintaining a proper posture, I still feel extra tense over my low back. Therefore, I went to see my chiropractic colleague and got a chiropractic adjustment. My back felt much better after the treatment. The stress is less, my back does not feel locked up, and the muscles feel less tense. I am going to take things easy the next few days. I will be doing some gentle low back stretching exercises and get chiropractic adjustments again to make sure my spine is in balance. Chiropractic care is safe and effective for back pain on top of doing stretching exercises and watching your posture.

Sitting time and all-cause mortality risk | Bellevue Chiropractor

Too much sitting linked to shortened lives: study(2)


         People who spent a lot of time sitting at a desk or in front of a television were more likely to die than those who were only sedentary a few hours a day, according to an Australian study that looked at death rates during a three-year period. (1) Researchers, whose results appeared in the Archives of Internal Medicine, found that the link between too much time sitting and shortened lives stuck even when they accounted for how much moderate or vigorous exercise people got, as well as their weight and other measures of health. The study concludes that prolonged sitting is a risk factor for all-cause mortality, independent of physical activity. Public health programs should focus on reducing sitting time in addition to increasing physical activity levels. There is also another study found out that time spent in sedentary behaviors was positively associated with mortality, and participation in high levels of moderate vigorous physical activities did not fully mitigate health risks associated with prolonged time watching television.(3)

      In short sitting too long will shorten your lifespan even if you do exercises regularly. In order to live longer and healthier it is very important for us to take mini breaks at work or when watching TV. A simple solution is to get up and move around for a few minutes every hour. You can use a timer to remind yourself. Stretching your neck, upper back and low back will keep you body flexible and prevent you from foot pain, forward head posture, neck pain, back pain, or work injuries. As a chiropractor I always recommend my patients to do regular stretching and posture exercises at work and home. It takes about 10 minutes but you will feel much better over you back and neck right away. Don't wait until it is too late. Besides stretching, or walking around, it is also imperative to have your spine checked by a chiropractor. In this fast pace society and with sedentary life style, we are putting lots of stress on our body without knowing it. Seeing a chiropractor can keep your spine active and healthy and in balance. If your spine goes out of balance, it will put pressure on your body. You will have aches and pain. Chiropractic adjustment is a natural and safe way to keep you in balance! We help you stay active and you will live longer!

I went to a seminar about Auto Collision Related Spine Injuries 2012 II by Dr. Dan Dock, DC on Thursday, March 8th, 2012. Here I will share some of the information that I learned from the seminar. Cranial nerve I to XII function and auto collision injuries were the topic. There were lots of information about cranial nerve evaluation and it's clinical relations. It really reminds me the good old days in the neuroanatomy and neurology class in school. The seminar was very in depth and felt like attending a neuro diplomat class. 

Dr. Dock also talked about Sidney Crosby's concussion like symptoms. Concussion like symptoms such as his dizziness and blur vision is a result of neck muscle strain/tightness.1, 2 Post trauma vertigo may be due to vasomotor instability.2 But, may be due to vestibulospinal system or chronic neck spinal muscle tightness. Cervical vertigo can be a result of dizziness from proprioception, vertebral artery irritation or sympathetic irritation. Chiropractic adjustment can help condition like this. Dr. Dock says that we are already helping people with similar condition in our daily practice even though you are not treating Sidney Crosby. You might have heard patients say that they can see better after cervical adjustment. The neurological pathway after cervical adjustment is that it stimulate the proprioceptor in the spinal cord. The information goes to the cerebellum via medial longitudinal fasciculus tract. It then goes to to CN III, IV, and VI that helps with vision.  

There are other diseases causing blur vision and dizziness such as ALS, MS, stroke and aneurysm. These are contra-indication for chiropractic treatment. In the seminar he went over signs and symptoms of many disease related to cranial nerves. It was quite a morning re-learning neurology. I had fun!

Auto Collision Related Spine Injuries 2012 Part I


Hi everyone~ I went to a seminar about Auto Collision Related Spine Injuries 2012 by Dr. Dan Dock, DC on Thursday, Feb 9, 2012. Here I will share some of the information that I learned from the seminar. As far as record keeping is concerned. Dr. Dock recommends hand writing notes. It has to be readable by other people as well. He also focused on incorporating an exercise program for patients to do in the office for their neck and/or back injuries.

There are ways to show the treatment is reasonable and necessary. Such as how patients activity of daily living have improved since getting care. Please be specific such as on the right or left, and which body part. And if their pain level have changed. You can use none, minimal, mild, moderate or severe to rate pain level besides using 0 to 10 scale. Mechanism of injury is the key to find out injury and to see clinical correlation. Where was the car hit? Where was the head turn? Was the foot on the break? Were arm on the steering wheel? If hit from behind, patient's body go backwards then forwards. If patient could not recall his position mostly he has light concussion, or short term amnesia. If the amnesia last for more than 2 to 4 hours, patient needs a CT scan. There might be intercranial bleeding.

If patient has gradual onset of pain following the accident. Most likely because the endorphin is slowly wearing out after the accident. If patient did not come for treatment, ask why. Is it because work, job, family or no health insurance? Look for muscle spasm and rate the level. Check for Active range of motion and passive range of motion once a month. Decreased cervical extension could be pedicle or facet fracture. Asymmetrical loss of motion could mean injury to the facet and disc. Look for sharp angulation, sharp reverse curve in cervical flexion and extension x-ray. This could mean damage to the capsule ligament, facet joint, interspinous ligament, and supraspinous ligament. If there is breakage in George's line there might be torn disc or fracture.

MRI for patient if there is distinct nerve root pattern, intense pain, something new and different, or patient is getting worse with treatment. Consider pre and post x-ray if the condition is unstable.

Care plan: update frequency of care, duration of care, exercise program, physical limitation and referrals for second opinion. Supportive care means patient's condition deteriorates without care. Maintenance care: no change in subjective and objective findings without care.

What is an effective treatment for neck pain: Chiropractic adjustment, exercises or medication?
